One serious injury has been reported.<br /><br />The adapters are used as accessories in a variety of respiratory applications. If they are blocked or occluded, exhalation or inhalation could be prevented, the company said.<br /><br />A safety alert that was issued earlier this week, listing two Hospitak lot numbers for the 22-millimeter/15-millimeter adapters, was extended after investigation. The recall includes other lots of the product, including adapters used in other devices. Some of the products have been labeled and sold under the names Viasys, Unomedical and Drager.<br /><br />Unomedical said patients and health care providers using Hospitak, Viasys, Unomedical or Drager airway adapters should check with Unomedical at 1-800-634-6003. Unomedical said it is working with the FDA, and people with further questions could contact their local FDA office for more information.]]></content:encoded>

Several adapters have been found to be blocked or occluded - potentially preventing exhalation or inhalation. This could result in serious or life-threatening injury to patients.<br /><br />In a public alert issued on November 28, the company warned the public that at least two lots of its Hospitak brand 22mm/15mm adapters may have been affected by this problem. Although this product is primarily distributed to medical institutions, some units ma y be distributed for home use. These adapters are used in a wide variety of respiratory applications, for example: extending breathing circuits and attaching reservoir bags. Consumers who have one of these recalled products should stop using it, contact their physicians and contact Unomedical, Inc.<br /><br />Subsequent investigation by the company indicates that other lots of the product, including airway adaptors used in other medical devices, may have been affected. Several adapters have been found to be blocked or occlude, potentially preventing exhalation or inhalation. This could result in serious or life-threatening injury to patients<br /><br />To date one serious patient injury has been reported.<br /><br />Although this product is primarily distributed to medical institutions, some units could be distributed for home use. These adapters are used as accessories in a wide variety of respiratory applications, for example: extending breathing circuits and attaching reservoir bags.<br /><br />Hospitak brand 22 mm /15mm adapters were distributed to hospital and home health care supply distributors nationwide.
