Traumatic Bleeding from Pradaxa

Pradaxa (dabigatran) is a blood thinner that was approved by the U.S. Food & Drug Administration (FDA) in October 2010 to prevent blood clots in patients with atrial fibrillation. Like all blood thinners, Pradaxa can cause serious bleeding. However, bleeding from warfarin, the decades-old blood thinner Pradaxa was meant to replace, can be stopped via the administration of vitamin K. There is no simple antidote for Pradaxa bleeding.

The elderly are much more vulnerable to serious traumatic hemorrhaging from Pradaxa. For one thing, they are more likely to suffer from balance problems, arthritis and other disorders that make them susceptible to falls that can cause minor traumas that result in Pradaxa intracranial hemorrhage and other serious bleeding. Because the kidneys of older people may not filter efficiently, Pradaxa can stay in their system longer. For these patients, dialysis may be needed to clear Pradaxa from their system in time to stop serious traumatic bleeding.

In March 2012, a case study in the Journal of Neurosurgery highlighted the dangers of Pradaxa traumatic bleeding faced by this patient population. The report detailed the worsening condition, and ultimate death, of an 83-year-old man at the University of Utah Medical Center. At first, the man’s case appeared to be that of a minor fall. Initially the patient was alert, and his neurological exam produced no concerns. CT scans revealed only small, superficial areas of hemorrhaging in his brain.

However, within two hours of his arrival at the hospital, new scans showed extensive progression of intracranial hemorrhaging. His doctors administered intravenous fluids and a protein called recombinant factor VIIa to stop his brain bleeding, but nothing worked. Dialysis was also considered to clear Pradaxa from the man's system, but he deteriorated too rapidly. The man fell into a deep coma, and died shortly after.
