FDA Approval & Action Timeline

  • March 2004 – Gilead Sciences, Inc. submits FDA applications for its new TDF drug called Viread.

What Is the Problem With Tenofovir Disoproxil Fumarate (TDF)?

Gilead Sciences Inc’s TDF prescription medications such as Truvada, Atripla, Viread, Stribild, and Complera have been linked with severe side effects such as bone density loss with injuries and kidney damage. In April of 2019, numerous lawsuits were filed against Gilead Sciences Inc. alleging the company knew about the serious side effects in 2001, created a newer medication that was safer, and continued to market TDF drugs as effective without adequate warnings to patients.

HIV Medication Linked to Serious Side Effects

(08/23/2019 – Update) NORTH CAROLINA – A drug used to treat HIV and meant to help prevent the virus from multiplying within a person’s body is the subject of over one hundred lawsuits filed across the nation. The drug at issue is tenofovir disoproxil fumarate, or TDF. TDF is found in medications such as Truvada, Viread, Stribild, Atripla, and Complera. According to the lawsuits, patients who take TDF may experience an accumulation of lactic acid in their bloodstream as well as age long-term harm to the kidneys and to bones.

The History of TDF and Newer Alternative Treatments

TDF received approval from the Food and Drug Administration (FDA) in 2001 to be used in conjunction with other medications to treat HIV. The TDF-based treatment protocol was replaced in 2015 with a new treatment protocol using the drug tenofovir alafenamide fumarate (TAF). The TAF-based treatment protocol appears to have fewer potential side effects and have a smaller dosage. However, according to the lawsuits, TDF manufacturer Gilead Sciences “slow-walked” the research and development of TAF drugs so that they could continue to make a profit off of TDF medication. The lawsuits further claim that Gilead knew that TDF presented risks of potentially life-threatening complications and that TAF was a safer alternative but nonetheless stuck by the more dangerous TDF.

What Harm Does TDF Cause for Patients?

The lawsuits’ main contention, then, is not that Gilead did not warn consumers of TDF’s side effects but rather that Gilead did not do enough to make TAF medications available. The harm that TDF can cause to patients are not new: in addition to minor side effects like diarrhea, nausea, headache, and the development of a rash, TDF can, over time, lead to liver and kidney problems. A patient who has preexisting kidney troubles and who takes TDF may be at risk of kidney failure. In addition, the drug may lead patients to experience a loss of bone density when the drug is taken over time.

Tenofovir Linked Bone Disease & Fractures

In May of 2017, the U.S. National Library of Medicine National Institutes of Health published a review that detailed the results of studies conducted to determine the short-term and long-term health effects of Tenofovir used in antiretroviral therapy (ART). The review’s goal was to “describe potential mechanisms underlying the adverse effects of tenofovir on bone,” and to perform “clinical studies of tenofovir disoproxil fumarate (TDF) and bone.”

The review also conducted an examination of tenofovir alafenamide (TAF) to determine if this drug was effective and was safer that TDF drugs. The review found a “60% increased fracture risk in HIV-infected individuals when compared to uninfected individuals.”

Tenofovir Linked to Acute Renal Failure (ARF) and Chronic Renal Failure (CRF)

In October of 2013, the Journal of the American Society of Nephrology (JASN) published a report called, “Tenofovir Effect on the Kidneys of HIV-Infected Patients: A Double-Edged Sword?” which examined the “potential nephrotoxicity that tenofovir would have in common with two other drugs from the same family.”

The reported that the first documented case of “TDF-induced acute nephrotoxicity” had been published back in 2001 and since 2001 there have been many other published cases. The JASN report also concluded that there is a link between TDF and an increased risk of tubular toxicity. Tubular toxicity leads to acute tubular necrosis and studies have shown that when a patient is taken off of TDF drugs, the condition reverses itself within a few months and kidney function improves.
The report published by JASN reported that other renal diseases such as Acute Renal Failure (ARF) and Chronic Renal Failure (CRF) are linked in some studies while other studies have a more optimistic view of study results claiming, “ long-term use of TDF is safe for the kidneys.”

TDF Litigation in the News

  • May 2018 – A class action lawsuit was filed by Devin Martinez and Ricardo Wohler against Gilread Sciences, Inc. in the Supreme Court of the State of California (Los Angeles County). This class action lawsuit was filed on behalf of Californians prescribed a TDF medication between October 26, 2001, and present.
  • April 2019 – A lawsuit was filed against Gilread Sciences, Inc. in San Mateo County Superior Court. The lawsuit was brought by 50 patients who claim the TDF drugs manufactured by Gilread Sciences, Inc. exposed them to severe side effects such as bone fractures and kidney diseases.
