Antiretroviral Prophylaxis Drug Side Effects Lawsuit. Several antiretroviral drugs taken by pregnant women to prevent the transmission of HIV to their unborn children may increase the risk that a baby will be born with a cleft palate or cleft lip. Research published in January 2012 raised red flags over several antiretroviral drugs and these birth defects, including:

  • Sustiva (efavirenz)
  • Epivir (lamivudine)
  • Trizivir (a combination of abacavir sulfate/ lamivudine/zidovudine)
  • Viracept (nelfinavir)
  • Viramune (nevirapine)
  • Zerit (stavudine)
  • Kaletra (lopinavir/ritonavir)
  • Combivir (lamivudine/zidovudine)

Anti-HIV Drugs and Oral Clefts

Antiretroviral drugs have been able to decrease the risk of HIV transmission from mother to child from to less than 1%. However, despite their widespread use among expectant mothers with HIV, little is known about their effects on a developing fetus. In January 2012, a study published in Cleft Palate-Craniofacial Journal investigated the association between antiretroviral prophylaxis and cleft lip and palate birth defects. The authors of the study examined 5 years of data from the U.S. Food and Drug Administration’s Adverse Events Reporting System (AERS). They found that in that time period, eight antiretroviral drugs were associated with 27 reports of cleft lip or cleft palate:

  • Sustiva (efavirenz) – 6 reports
  • Epivir (lamivudine) – 5 reports
  • Trizivir (a combination of abacavir sulfate/ lamivudine/zidovudine) -2 reports
  • Viracept (nelfinavir) – 2 reports
  • Viramune (nevirapine) – 3 reports
  • Zerit (stavudine) – 1 report
  • Kaletra (lopinavir/ritonavir) – 5 reports
  • Combivir (lamivudine/zidovudine) – 3 reports

The study authors concluded Sustiva showed the strongest medication link to cleft lip and palate, followed by Epivir, Viracept, and Viramune. Among combination therapies, Trizivir had the strongest link, followed by Kaletra and Combivir. The authors called for more research to determine if there is a link between antiretroviral drugs and cleft lip and palate.
